Excellence in Patient Care: Humboldt Park Health’s Integrated Approach

For over 125 years, Humboldt Park Health has actively provided quality, affordable, and compassionate healthcare on the west side of Chicago. As a safety net hospital, it serves a primarily underserved population and advances health equity for all. Under José R. Sánchez’s leadership, the hospital has navigated numerous challenges and transformed its services to meet the changing demographics and needs of its patients.

Humboldt Park Health offers a comprehensive range of healthcare services, including emergency services, primary care, specialty care clinics, preventive screenings, diagnostic imaging, laboratory services, and various wellness programs. The hospital focuses on delivering integrated, patient-centered care that addresses both physical and mental well-being.

Moreover, the hospital serves as an educational facility, hosting Family Medicine and Podiatry residency programs. It provides 70-75 medical students with the opportunity to train within the community. By investing in a residential building for its trainees, Humboldt Park Health enables them to complete their residency training while living and practicing in the Humboldt Park community.

To address the community’s social determinants of health, Humboldt Park Health initiated the development of a 45,000-square-foot Wellness Center, scheduled to open in January 2025. This facility will offer amenities such as an indoor running track, swimming pools, rehabilitative services, and mind-body integration programs, serving as a family destination for holistic wellness.

The hospital has also made significant capital investments to improve patient care through new technologies. It established new CT and 3D imaging mammography suites, a Physiological Monitoring System, and a new MRI suite. These advancements ensure that the community receives the same level of services available in more affluent areas. Humboldt Park Health leverages advanced electronic health records with their new single enterprise EHR, MEDITECH Expanse, remote monitoring devices, and data analytics to enhance efficiency, access, and patient experience.

Additionally, Humboldt Park Health developed the HPH Connect App to streamline the physician referral process and enhance communication between physicians, providers, and patients. This app exemplifies how Humboldt Park Health uses technology to improve the patient and physician experience, ensuring that the community receives the best possible care.
