Dr. Marc Ruel: The Surgeon Who Redefined How the World Reaches the Heart

Some innovations arrive with fanfare. Others arrive quietly, changing a profession so thoroughly that people eventually forget there was ever another way. For decades, cardiac surgery was lifesaving but inherently invasive. Surgeons around the world continually searched for ways to make procedures less traumatic for patients while preserving the precision and reliability upon which lives depend. It was a challenge that required not only extraordinary technical skill but also imagination.

Dr. Marc Ruel possessed both. Today, as Professor, Director of Minimally Invasive Cardiac Surgery, and Endowed Chair of MICS Research at the University of Ottawa Heart Institute, Dr. Ruel stands among the most influential figures in modern cardiovascular medicine. Internationally recognized as the pioneer of multi-vessel minimally invasive coronary artery bypass surgery (MICS CABG), he has helped reshape how surgeons approach some of the most complex procedures in cardiac care.

Yet his remarkable career is not defined by a single innovation. It is defined by an extraordinary ability to influence surgery, research, education, policy, and leadership simultaneously, creating a legacy that extends far beyond any one operating room.

Transforming Practice Through MICS CABG

Medicine advances when someone refuses to accept that the current standard is the final answer. For Dr. Ruel, that mindset has guided more than two decades of clinical and academic work at the University of Ottawa Heart Institute, where he has served as a cardiac surgeon since July 2002.

His work emerged from a straightforward but ambitious objective: achieving exceptional surgical outcomes while reducing the physical burden placed on patients. Heart surgery has always demanded courage from those who undergo it. His innovation sought to lessen how much of that courage was required.

The development of multi-vessel MICS CABG became a defining milestone in that pursuit. The procedure demonstrated that complex coronary artery bypass surgery could be performed through minimally invasive approaches without compromising quality or outcomes.

What began as a surgical innovation eventually became a global movement. Today, MICS CABG is widely adopted around the world, influencing the way cardiac surgeons think about access, recovery, and patient experience.

Yet he never treated innovation as something to be guarded. Instead, he became a teacher to the global surgical community.

As a visiting professor at institutions across multiple continents, he has performed cardiac surgeries, shared expertise, and trained hundreds of heart surgeons from every continent. In medicine, knowledge has a unique ability to multiply. Every surgeon who learns a technique carries that knowledge forward to future patients, creating an impact that extends far beyond a single practitioner. Through education alone, his influence has reached operating rooms around the world.

Building Institutions That Grow Stronger

The most effective healthcare leaders improve not only patient outcomes but also the organizations entrusted with delivering them.

From January 2013 to October 2023, Dr. Ruel served as Professor, Head, and Chair of the Division of Cardiac Surgery at the University of Ottawa Heart Institute.

During his 11-year tenure, the cardiac surgery program experienced substantial growth in surgical volumes, quality initiatives, research activity, innovation, advanced education, national outreach, external referrals, international reputation, and global recruitment.

Such progress rarely happens by accident. It requires a leader capable of balancing clinical excellence with strategic vision, while creating an environment where talented people can thrive.

In October 2023, he assumed his current position as Professor, Director of Minimally Invasive Cardiac Surgery, and Endowed MICS Research Chair, continuing his commitment to advancing the future of cardiovascular care through both innovation and research.

His leadership has consistently reflected a belief that institutions, when thoughtfully guided, can amplify the impact of individual achievements.

A Scholar Defining Modern Cardiac Surgery

The future of medicine is often written long before it reaches the bedside. It begins in research papers, clinical studies, textbooks, and scientific discussions that gradually shape how physicians think and practice. Dr. Ruel has played an extraordinary role in that process.

Over the course of his career, he has published more than 500 scientific papers and chapters, authored seven books, and delivered more than 400 invited lectures, most of them on international platforms.

His textbook on Cardiac Surgical Techniques, now in its third edition and published in five languages, has become an important educational resource for surgeons worldwide.

These accomplishments are impressive in their scale, but their significance lies in their reach. Each publication contributes to a growing body of knowledge. Each lecture shares expertise with colleagues. Each book helps prepare future generations of surgeons.

Together, they represent a lifelong commitment to advancing the science behind cardiovascular care.

Advancing Evidence Through Research

Innovation achieves its greatest value when it is supported by evidence. Dr. Ruel has remained deeply committed to ensuring that the future of cardiac surgery is guided by rigorous scientific inquiry.

He currently leads important international clinical trials, including MIST and ODIN, studies designed to answer critical questions about the next generation of cardiovascular treatment strategies. Clinical trials require patience, precision, and a willingness to invest years pursuing answers that may ultimately benefit patients across the world.

That long-term perspective has become a hallmark of his career. His work reflects a belief that meaningful progress in medicine is built not only on bold ideas, but also on careful validation.

Shaping Cardiovascular Leadership Worldwide

Dr. Ruel’s influence extends well beyond clinical practice and academic research.

Since January 2020, he has served as Director of The Society of Thoracic Surgeons, one of the most respected professional organizations in the field.

Since 2019, he has also served as Director of the International Society for Minimally Invasive Cardiothoracic Surgery, helping guide the ongoing evolution of minimally invasive techniques across the global surgical community.

From October 2020 to October 2022, he served as President of the Canadian Cardiovascular Society (CCS), an organization established in 1946 and recognized among the world’s leading cardiovascular societies. His presidency proved transformative.

Under his leadership, the CCS secured its first federal funding, launched its first public advocacy initiatives, established its first Equity, Diversity, and Inclusion Committee, created the Cardiovascular Centres’ Alliance, and introduced a leadership code of conduct.

These accomplishments demonstrate a leader who understands that advancing healthcare requires more than scientific discovery. It also requires stronger institutions, broader collaboration, and thoughtful governance.

A Trusted Voice in Global Cardiovascular Medicine

The medical community’s trust in Dr. Ruel’s expertise is reflected in the many responsibilities he continues to hold.

Since November 2020, he has served as Associate Editor and Cardiovascular Surgery Editor for Circulation, the premier cardiovascular scientific journal published by the American Heart Association.

In addition, he has participated in or led 18 international practice guidelines and consensus statements, helping shape standards of cardiovascular care across healthcare systems worldwide.

His contributions continue to influence how physicians diagnose, treat, and care for patients far beyond his own institution.

The Legacy of a Visionary

Recognition inevitably follows sustained excellence, and among Dr. Ruel’s many honors is the Queen Elizabeth II Diamond Jubilee Medal.

Yet perhaps his greatest achievement cannot be measured through awards, publications, titles, or leadership positions. It can be found in the patients who recovered through less invasive procedures, the surgeons he has trained across continents, the institutions he has strengthened, and the scientific knowledge he has contributed to medicine.

Dr. Marc Ruel has not simply advanced cardiac surgery. He has expanded its possibilities. His career demonstrates that the most transformative innovations are not solely defined by technology or technique. They are defined by the lives they improve and the opportunities they create for others.

And in a profession dedicated to caring for the human heart, Dr. Ruel’s enduring contribution may be the way he has connected innovation, education, and leadership to make that care more effective, more accessible, and more far-reaching than before.
